Brief Overview of the PH Essential Services
  1. Assessment – monitoring, investigating, and addressing population health conditions, hazards, and root causes.
  2. Policy Development – Creating, strengthening, and championing the necessary policies, procedures, and partnerships to ensure effective communication, collaboration, and program implementation.
  3. Assurance – Building and maintaining a strong, innovative, diverse, and equitable public health workforce and infrastructure.

Community Health Assessment

A Community Health Assessment Advisory Group participated in the identification and analysis of health needs in Tehama County. Assessment of population health requires analyzing a broad range of the social, environmental, and behavioral factors that contribute to disease, injury, and death. A comprehensive Community health Assessment is the call to action
for prioritization and improvement planning.

Community Health Improvement Plan

A collaborative, stakeholder-driven effort to enhance the health and well-being of all residents. Following development of the Community Health Assessment, a diverse committee of representatives including community members, equity groups, healthcare and education professionals, and the business community provided insights and knowledge to identify critical health challenges and opportunities for improvement. Through shared ownership and accountability, the Tehama Community Health Collaborative was formed to address the three priority areas of healthcare access, substance use, and access to food.

Public Health Strategic Plan

Drawing on efforts and insights in the development of our Community Health Assessment and Community Health Improvement Plan, our Strategic Plan provides the roadmap for our department to realize our goal of a thriving, united, and healthy community. With input from line staff, supervisors, upper management, and advisory board representatives, the plan demonstrates a commitment to strengthening our capabilities in assessment, assurance, and policy to better meet the identified needs of the community and strengthen our capabilities in alignment with the nationally recognized 10 essential services for public health.

PH Pathways

National Public Health Accreditation advances standards of excellence and accountability for the systems that promote and protect the health of all people. Tehama County Public Health seeks Pathways Recognition from the Public Health Accreditation Board as a meaningful step along its journey to support performance improvement, strengthen infrastructure, and facilitate system transformation to be in alignment with our community health needs.
